2008 U.S. Customer Satisfaction Survey Sweepstakes

Official Rules

1.     NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THIS SWEEPSTAKES.

2.     Eligibility: Sweepstakes begins February 3, 2008 at 12:01:00 a.m. Eastern Time (ET) and ends January 31, 2009 at 11:59:59 p.m., ET.  Participation open to legal residents of the United States, age 21 or over and who are of legal age of majority in their jurisdiction of residence. Sweepstakes is offered only in the United States and is void in Alaska, Hawaii, Louisiana, Puerto Rico and where prohibited. Employees and Agents (and their respective family members: spouses, children, siblings, in-laws) of Staples Contract & Commercial, Inc., its parent, affiliates, officers, directors, agents, employees, subsidiaries and service agencies [or independent contractors] are ineligible to participate or win a prize. A PURCHASE DOES NOT INCREASE ONE’S CHANCE OF WINNING.

3.     To Enter: Participants may enter during each of Staples four (4) fiscal quarterly "Sweepstakes Periods,"(2/3/08 - 5/3/08); (5/4/09 - 8/2/08); (8/3/08 - 11/1/08); (and 11/2/08 - 1/31/09). There are two ways to enter:

(1) Customers who receive an official Survey Code can complete a customer satisfaction survey online or by phone during a Sweepstakes Period in order to enter for that quarter’s drawing. To complete survey online, log onto: http://webview2.isacorp.com/o9955/. To complete survey by phone, call toll-free 1-800-890-7643.  You will be automatically entered upon proper completion of the survey (note: survey participants have 30 days from the date they receive a survey code to complete the survey);

(2) Alternate Entry: No Purchase Necessary. Legibly hand print on a 3” X 5” postcard, your complete name, street address (no P.O. Boxes please), including city, state and zip code, daytime phone number (including area code) age and e-mail address (if available) and mail to: 2008 SBD Customer Satisfaction Survey Sweepstakes, P.O. Box 748, Georgetown, CT 06829-0748. All mail-in entries must be postmarked on or before the last day of the last month in a Sweepstakes Period and received by the 7th day of the immediately following month for entry into that quarter’s drawing. If the last day of a quarter and/or the 7th day of the immediately following month falls on a Sunday or a government Holiday, the entry may be postmarked or received (as appropriate) on the immediately following business day. Each postcard entry must be mailed separately. No mechanical reproductions or facsimiles of postcard entries will be accepted.

Limit one entry per household each month, regardless of method of entry. Entries received for a given quarter’s drawing will not be retained for subsequent month’s drawings. Only complete entry forms are eligible. In case of dispute as to identity of an online entrant, entry will be declared made by the authorized account holder of the E-mail address used to enter the Sweepstakes at time of entry. "Authorized Account Holder" is defined as the natural person who is assigned an E-mail address by an Internet access provider, online service provider, or other organization (e.g., business, educational, institution, etc.) responsible for assigning E-mail addresses or the domain associated with the submitted E-mail address. Any other attempted form of entry is prohibited; automatic, programmed, robotic or similar means of entry are not permitted. Sponsor and its promotion and advertising agencies are not responsible for lost, late, damaged, incomplete, illegible, misdirected or postage due entries/mail or for technical errors of any kind including computer/information network failures, hardware, software, telephone or other communications malfunctions of any nature, errors or failures of any kind, lost or unavailable telephone or network connections, Website, Internet, or ISP availability, unauthorized human intervention, traffic congestion, incomplete or inaccurate capture of entry information (regardless of cause) or failed, incomplete, garbled, jumbled or delayed computer transmissions which may limit a participant’s ability to take part in the Sweepstakes. All entries become the sole property of the Sponsor and will not be acknowledged or returned. CAUTION: Any attempt by an entrant to deliberately damage any website or undermine the legitimate operations of the Sweepstakes is a violation of the criminal and civil laws. Should such an attempt be made, the Sponsor reserves the right to seek damages from any such entrant to the fullest extent permitted by law and disqualify such entrant from the Sweepstakes.

4.     Drawing: Winners will be selected in a series of four (4) quarterly random drawings with one drawing (and one winner) to be held for each Sweepstakes Period; that is, one drawing in May 2008 (for Feb. 3 – May 3, 2008 Sweepstakes Period); one drawing in August 2008 (for May 4 – Aug. 2, 2008 Sweepstakes Period); one drawing in November 2008 (for Aug. 3 - Nov. 1, 2008 Sweepstakes Period); and the last drawing in February 2009 (for Nov. 2 - Jan. 31, 2009 Sweepstakes Period). Each quarter’s drawing will consist of all eligible entries received for that quarter (see Section 4 "To Enter", above). All drawings will be conducted by PHD Marketing Services LLC, an independent judging organization, on or about the fifteenth day of the month immediately following the Sweepstakes Period. Odds of winning depend on the total number of eligible entries received for each quarter’s drawing. Judges’ decisions will be final and binding in all matters relating to the Sweepstakes. Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify, or suspend this Sweepstakes in whole or in part for any reason in its sole discretion including, without limitation, to change the sweepstakes rules or prizes in any jurisdiction or if causes beyond its control corrupt or affect the administration, security, fairness or proper conduct of the Sweepstakes. In such case, Sponsor will select the winner from all eligible entries received prior to and/or after (if appropriate) the action taken by Sponsor or from mail-in entries only. By entering Sweepstakes, entrants agree to be bound by these official rules and the decisions of the judges.

5.     Prize: One (1) Grand Prize for the quarterly drawing: $2,500 (US) Staples merchandise coupon. Merchandise coupon is NOT redeemable, transferable or exchangeable for cash and cannot be applied to existing credit balances. If winner uses Staples coupon to purchase goods in excess of $2,500 winner shall be solely responsible for such amount in excess of $2,500. The prize awarded in connection with the Sweepstakes is nontransferable and non-endorsable. The prize (or any portion thereof) cannot be sold or transferred. No prize substitutions allowed, except at Sponsor’s sole discretion in the event of prize unavailability for any reason, and then only for a prize of equal or greater value. All federal, state and local laws and regulations apply.

6.     Notification: Each prize winner will be notified by phone, e-mail [if provided] and overnight mail. Winner will be required to sign and return an Affidavit of Eligibility and a Liability and Publicity Release (where legal) within 10 days of notification. If winner cannot be contacted, if prize is returned as undeliverable, or in the event of noncompliance with these Official Rules, the prize will be forfeited and an alternate winner will be selected. Upon forfeiture, no compensation will be given. Prize will be awarded to winner or an alternate winner under all circumstances.

7.     Conditions: All taxes are the sole responsibility of the winner accepting the prize. A 1099MISC Federal tax form will be issued to the winner for the value of the prize. Participation in Sweepstakes and acceptance of prize constitutes winner’s permission to use his/her name, address (city and state only), likeness and any statements made by winner and/or company regarding the Sweepstakes or Sponsor for advertising and promotional purposes without additional compensation, except where prohibited by law. By acceptance and use of prize, winner agrees to hold harmless and indemnify Sponsor and its parent organization, affiliates, officers, directors, agents, employees, subsidiaries and service agencies [or independent contractors] against any and all liability, damages or causes of action (however named or described), with respect to or arising out of entrant’s participation in the Sweepstakes, or the receipt or use/misuse of the prizes awarded herein. These parties, their respective affiliates and subsidiaries, advertising/promotional agencies, and each of their directors, officers, employees, agents, successors and assigns have no liability or responsibility for any personal injuries, losses or damages or claims of any kind resulting from participation in the Sweepstakes, acceptance, possession or use/misuse of any prize. Entrants who do not comply with the Official Rules or attempt to interfere with this Sweepstakes in any way shall be disqualified. There is no purchase or sales presentation necessary to participate.

8.     Additional Terms. Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds, in its sole discretion, to be tampering with the entry process or the operation of the Sweepstakes or Web site. For details regarding collection of information from users of the Web site (including entrants), please consult the privacy policy on the Web site. Additionally, Sponsor reserves the right to prosecute any fraudulent activities to the full extent of the law.

9.     WINNERS LIST. For the name of the quarterly prize winner, send a self-addressed stamped envelope, postmarked no later than the 15th of the month following the end of the quarter to: 2008 SBD Customer Satisfaction Survey Sweepstakes [specify quarter] Winners List, P.O. Box 746, Georgetown, CT 06829-0746. Residents of VT may omit return postage.
